Abstract: Ebstein disease is a congenital heart abnormality characterized by abnormal low insertion to the right ventricular apex of the tricuspid valve. It is often associated with an atrial septal defect, in which case there is a communication between right and left cavities of the heart that favours paradoxical embolism, an important risk factor for stroke. We will present the case of a 25-year-old after-birth female patient, known for Ebstein disease, and osteum secundum septal defect with right-sided hemiparesis and mixed aphasia. Cerebral CT was performed and it revealed left-sided silvian ischemic stroke. Paraclinical explorations and the presence of the stroke without an obvious embolic source in the arterial circulation, and of the right-left shunt cardiac defect, direct the diagnosis to paradoxical embolism. Cardiac failure has become symptomatic in the young adult and has caused brain complications. Surgical closure of the shunt is required to prevent paradoxical embolic events.

Title: Psychological disorders in relation to pain syndrome in patients with colorectal cancer

Abstract: Aim. The principle aim is to investigate the relationship between psychological distress, pain syndrome and clinical features in colorectal cancer patients. Methods. The study assessed forty-nine patients undergoing chemotherapy from Cancer Institute “Ion Chiricuta” Cluj-Napoca admitted between 2016 and 2017 with locally advanced or metastatic colorectal cancer. For each patient, pain intensity was evaluated using a visual analogue scale, whereas depression and anxiety levels were evaluated using Beck Depression and State-Trait Anxiety Inventories. All evaluations were determined at their first presentation in the hospital before treatment initiation. Correlations between demographic characteristics, pain evaluation and mental disorders were realized using Chi-squared tests (χ2) and Spearman’s rho. Results. Pain was observed in 76% of patients, 54% of whom had moderate to increased pain. Twenty-nine percent of patients presented with nociceptive pain, whereas 20% had neuropathic pain. In 27% of patients, the pain was mixed. Depression was moderate to severe in 53% of patients, and high levels of anxiety were found in 24% of patients. Levels of anxiety and depression did not correlate with demographic characteristics. There were correlations between the pain type and its level. Neuropathic pain correlated with moderate pain, while mixed pain was correlated with intense pain (p=0.0003). Tumor site was correlated with level of anxiety: colorectal cancer was associated with mild anxiety, rectal cancer was associated with moderate anxiety, and patients with anal canal cancer presented increased anxiety (p=0.026). Conclusion. High pain frequency is associated with high anxiety and depression levels in colorectal cancer patients. Pain intensity is correlated with pain type, while psychological disorders are correlated with tumor site.

The ultrasound exam has been a major contribution to the assessment and staging of patients with rheumatoid arthritis, is superior to the clinical and radiological examination for synovitis, an important role in therapy and the definition of remission of patients with rheumatoid arthritis.

Abstract: Smoking and alcohol use have always been risk factors for various disease such as pulmonary and liver disease, cancer but also have high interdependency with the generation of genetic anomalies such as cleft lip and palate. A strong connection has been set between the use of alcohol and tobacco and the severity of the cleft. We have made a study containing 139 patients with clefts hoping to mark the impact that smoking and alcohol consumption, but also other genetic factors have on the development and severity of clefts. Data collection was done through the anamnesis, through a series of questions regarding vicious behavior. The patients included in the study present a large variety of clefts ranging from simple cleft lips to complex labio-maxilo-palatal clefts. Risk factors have been assessed individually but most importantly in conjunction with other risk factors present in our study. While not the only risk factors present in our study, smoking and alcohol consumption during pregnancy have a large impact on the complexity of the clefts.
